Part ~

The TV can watch ah ? ESPN ?
Yes & Why ?
" Nothing ! I just want to confirm can watch World Cup or not ······· "

World Cup FootBall ?

3 Days / 2 Nights & Type of Rooms
Single Room = RM 2650
Two Bedded Room = RM 1850
General Ward = RM 1650

A comprehensive package inclusive of :
* Nursing Care for Mother & Baby
* Mother's Room & Nursery
* Labour Room
* Routine Medication
* Foetal Monitoring
* Care before and after delivery
* Delivery Charges
* Baby Care by Child Specialist
* Routine Blood Tests & Routine Injection for the baby ( Vit K, BCG & Hepatitis
* B - 1st dose )
* Maternity Diet
* Neonatal Hearing & Screening

This package does not include the following and will be charges accordingly :

* Minor Surgery and Blood Transfusion

** A full deposit is required during admission or the next day.
** All prices quoted are subject to change without prior notice.
** Charges are the same on Public Holidays and Weekends.

Finally,

W.5 - Part I b

Manual Expression of BreastMilk ( Marmet Technique )
The Marmet Technique of Manual Expression and assisting the milk ejection reflex ( sometimes called the let-down reflex ) has works of hundreds mothers - in a way that nothing has before.
Even experience breastfeeding mothers who have been previously been able to express only a small amount, or none at all, get excellent results with the technique.

Technique is Important
When watching manual expression the correct milking motion is difficult to see. In this case the hand is quicker than the eye.
Consequently, many mothers have found manual expressing difficult - even after watching a demonstration or reading a brief description.
Milk can be expressed when using less effective methods of hand expression.
When used, however, on a frequent and regular basis, these methods can easily lead to damaged breast tissue, bruised breasts, and even skin burns.

The Marmet technique of manual expression was developed by a mother who needed to express her milk over a extended period of time for medical reasons.
She found that her milk ejection reflex did not work as well as her baby breastfed, so she also developed a method of massage and stimulation to assist this reflex.

The key to the success of this technique is the combination of the method of expression and this massage.
This technique is effective and should not cause problems.
It can easily be learned by following this step by step guide. As with any manual skill, practice is important.

Advantages
The are many advantages to manual expression over mechanical methods of milking the breasts :
* Some mechanical pumps cause discomfort and are ineffective.
* Many mothers are more comfortable with manual expression of breast milk
* because it is more natural.
* Skin to skin contact is more stimulating than the feel of plastic shield.
* So manual expression usually allows for an easier milk ejection reflex.
* It's convenient.
* It's ecological superior.
* It's portable, how can a mother forget her hands ?
* Best of all it's Free !

How the Breast makes Milk ?
The milk is produced in milk producing cells ( Alveoli ).
A portion of the milk continuously comes down the ducts and collects in the milk reservoirs.
When the milk producing cells are stimulates, they expel additional milk into the ducts system ( Milk rejection reflex or let-down reflex )

Expressing the Milk
Draining the Milk Reservoirs
Use this measurement, which is not necessarily fingers outer edge of the areola, as a guide.
The areola varies in size from one woman to another.
* - Place the thumb above the nipple and the fingers below, in a 12 o'clock and
* - 6 o'clock position, as shown.
* - Note that the fingers are positioned so that the milk reservoirs lie beneath
* - them.
* - Avoid cupping the breast. The fingers should NOT be in 12 o'clock and 4
* - o'clock position.

* Position the thumb and first two fingers about 1" to 1 1/2" behind the nipple.

* Push straight into the chest wall. Avoid spreading the fingers the fingers
* apart. For large breast, first lift and then push into the chest wall.

* Roll thumb and fingers forward as if making thumb and fingers at the same
* time.

* Finish Roll - The rolling motion of the thumb and fingers compresses and
* empties the milk reservoirs without hurting sensitive breast tissue.

The arrows show the direction of the pressure the fingers apply.
See that the fingers under the breast start with the pressure on the back most finger in the " Push " and moves to the front most finger in the " Roll ".

Repeat Rhythmically to drain the reservoirs.
Position, Push, Roll, Position, Push, Roll ······ ········

* Rotate the thumb and finger position to milk the other reservoirs.
* Use both hands on each breast, the fingers will move from the 12 o'clock and
* 6 o'clock positions, to 11 and 5 o'clock, 2 and 8 o'clock, 3 and 9 o'clock.

Avoid These Motions
* Avoid Squeezing the breast. This can cause bruising.
* Avoid Pulling out the nipple and breast. this can cause tissue damage.
* Avoid Sliding on the breast. This can cause skin burns.

Assisting the Milk Ejection Reflex
This will help with relation and will help stimulate the milk ejection reflex.

Procedure
This procedure should be followed by mothers who are expressing in place of a full feeding and those who need to establish, increase or maintain their milk supply when the baby cannot breastfed.
* - Express each breast until the flow of milk slows down.
* - Assist the milk ejection reflex ( Massage, Stroke, Shake ) on the both breasts.
* - This can be done simultaneously.
* - Repeat the whole precess of expressing each breast and assisting the milk
* - ejection reflex once or twice more.
* - The flow of milk usually slows down sooner the second and third time as the
* - reservoirs are drained.

* Massage the milk producing cells and ducts. Start at the top of the breast.
* Press firmly into the chest wall.
* Move fingers in a circular motion on one spot on the skin.

* Stroke the breast area from the top of the breast to the nipple with a
* tickle-like stroke.

* Shake the breast while leaning forward so that gravity will help the milk eject.

Timing
The Entire Procedure should take approximately 20-30 minutes.
* Express each breast 5-7 minutes
* Massage, Stroke, Shake
* Express each breast 3-5 minutes
* Massage, Stroke, Shake
* Express each breast 2-3 minutes

Note : If the milk supply is established, use the times given only as a guide.
Note : Watch the flow of milk and change breasted when the flow gets small.
Note : If little of no milk is present yet, follow these suggested times closely.

Demo ~

W.5 - Part I a

Advantages of BreastFeeding

For Baby
* Due to the anti-infective properties of breastmilk, breastfed babies tend to
* have less incidence of or less pronounced symptoms or ear infections,
* respiratory illness, allergies, diarrhea and vomiting.
* Due to the digestibility of breastmilk, breastfed babies are rarely constipated.
* The stools of breastfed babies are mild-smelling.
* SIDS ( Sudden Infant Death Syndrome ) is less common in breastfed babies.
* Breastmilk is constantly changing in its composition to meet the changing
* needs of the baby. It has the exact combination of protein, fats, vitamins,
* minerals,enzymes, and sugars needed for the human infant at various staged
* of his growth.
* Breastfed babies are constantly exposed to a variety of tastes through their
* mother's milk.
* Breastfed children are at less likely to need orthodontic work such as braces
* due to the unique sucking action required with breastfeeding. They also seem
* to have better overall dental health than formula-fed children. Children who
* were breastfed need speech therapy less often than those who were bottle-fed.
* IQ levels are average of 8 points higher in children who were breastfed.
* Adult daughters who were breastfed are at less risk of breast cancer.
* The bond between mother and child seems to be enhanced with
* breastfeeding.

For Mother
* Nursing immediately following delivery causes the uterus to contract
* lessening the risk of postpartum hemorrhage.
* The uterus of a breastfeeding mother shrinks to its pre-pregnancy size more
* quickly.
* Calories are burned while breastfeeding. It takes approximately 20 calories
* to produce an ounce of milk.
* Women who nurse their babies for at least 6 months lessen their chances of
* pre-menopausal breast cancer.
* Osteoporosis and cervical cancer are less common in women who breastfed.
* The return of fertility is delayed with breastfeeding.
* Breastfeeding is more economical than formula feeding.
* Breastmilk is always available, clean and at the right temperature.
* Many mothers feel a special satisfaction in knowing that they alone are
* meeting the nutritional needs of theirs babies.
* Adults who were breastfed have a lower risk of high cholesterol and asthma.

哇塞!我最顶不顺的是 很多准妈妈会问助产士这样的问题;

Napkin Layer?
Napkin Layer =

How do I know when to come into hospital ?
What happens in Labour ?
* The womb gets ready for labour in the few months of your pregnancy.
* You will have felt your womb have small contractions when it goes hard.
* These are call Braxton-Hicks contraction.

* " False Labour ". Sometimes it is difficult for you to tell when the
* contractions are Braxton-Hicks contractions of the start of your Labour.

* The Onset of Labour
* - More frequent and painful contractions
* - Water bag may burst ( if the water bag burst you must come into hospital
* - even if you have no pain )
* - " Show " – mucous with blood mixed in it. ( fresh blood is very abnormal.
* - Come into hospital immediately )

When should I come into hospital ?
If this is your first pregnancy and you live in Penang there is no need to rush and break the speed limit. Unless you have had problems during your pregnancy ( eg. A breech or twins ) there is no need to rush into hospital with the first few contractions.
* If you have a " show ", wait for other signs that you are in labour.
* Have a shower or a bath, trying watching the tv, get some company……
* when the contractions become together stronger it’s time to go in.

The explain and understand labour, doctors and nurses devide into 3 stages.
* 1st stage
* - is from the start labour until the cervix is fully open ( at 10cm )
* 2nd stage
* - is from when the cervix is fully open until the baby is born.
* 3rd stage
* - is from the time the baby is born till after the delivery of the after birth,
* - the placenta.

What should I expect when I am in Labour ?
What should I expect ?
* Given a private room
* Change into a gown
* Examined by a nurse
* Water bag ruptured if necessary
* Baby is checked by a CTG machine
* Move around
* Offered pain relief

Pain relief
Most mothers are worried about pain when they are in labour. Some of the way which you can try to help coup with the pain are :
* Massage by loving husband
* Breathing exercises
* Injections
* Breathing a gas called Entenox
* Epidural

What problem can happen ?
No one can guarantee very labour will be a normal labour. That is why you have doctors, nurses and monitors to look after you and your baby. Occasionally problems can happen in labour ?
* The cervix may stop opening up
* - A drip may be started to give pitocin to get the contractions stronger
* - A caesarean may be needed

* There might be fetal distress when the baby is not getting enough oxygen
* - A caesarean may be needed

* The mother needs some help to deliver
* - Forceps or Ventouse

Other problems…..
What is an episiotomy and why is it done ?
* A cut " down below "
* To prevent a bad tear
* Speed up labour if there has been no progress or if there is fetal distress
* In forceps deliveries
* It is not done routinely

What happens after the baby ?
Having a baby is only the beginning of many things. What happens after ? There are many things to consider.
* Your body begins to readjust itself
* - Get tone back onto your muscles
* - Pelvic floor exercises to reduce the risk of prolapsed and urinary
* - incontinence
* Breast feeding

Why should I breastfeed ?
* Protection against infections
* - Less diarroea and lung infections compared with the bottle feeding
* - because breast milk has antibodies

* Protection against allergies eg. Asthma
* - Even a single bottle of formula milk early on can risk of allergy

* Lower risk of cot death ( also know as SID, sudden infant death )

* An association with higher intelligence ! Because some of the nutrition that
* the baby’s needs during its early development cannot be imitated by
* powdered milk

* Emotional bonding with your child. Some mothers find it hard to love their
* babies. Breastfeeding helps mother and baby to start develop a loving
* relationship

* Makes night feeds and travelling more convenient

* No running out of supplies !

Breast milk is BEST !

Want another baby next year ?
Look around you at home or at the office.
* Condoms
* The " Coil " or IUCD
* The combine pill
* The mini-pill

Prevent Cervical Cancer
You will routinely be offered a cervical pap smear at your check-up after your delivery

After the baby…..
* Breast feeding
* Postnatal exercises
* Sex !
* Depression
* Hair loss
* Sleepless nights
* And many more…..
It is not easy to being a mother. Many mothers often say " I’m coming back as a man !"

Die Die must GO ~

W.2 - Part II

Is My Baby OKAY ?
During you pregnancy, your baby is cocooned and nourished within the mother's body. We can get fair idea of the baby's condition by monitoring his growth and activity in the mother's womb. However, it is not until the baby actually arrives that we can know his or her condition for sure.

When parents ask their doctor : " Is my Baby OKAY ?", they usually are interested to know whether the baby is normal, and free from any problems.

In this respect, the first few minutes after the delivery are crucial. The baby's first hurdle is to establish breathing quickly, and some babies announce their arrival by screaming at the top of their lungs, thereafter, a quick measurement of weight, lenght and head circumference will tell if the baby's size is within normal limits.

In most instanced, by the time the parents get to see and hold their body, he or she would have been bathed and dressed up. Still, it is not uncommon ( and quite natural ) to find parents gently examining every inch of newborn's features and making sure that there are really 10 fingers and 10 toes.

Appearance
A newborn baby continues to assume a curled-up position ( Foetal Position ) that he or she has been used to inside the mother's womb, he may have a thin layer of fine hair ( Lanugo ) on his body, and many Asia babies also have bluish-gray patches over the side and buttocks ( Mongolian Sports ), which will fade with time.

Many babies' heads will be moulded in an ablonged fashion ( Caput ) which help them negotiate the birth canal. Others may develop a swelling on one or both sides of the scalp ( Cephalhaematoma ) but virtually, all will resume a normal shape with time and without any help.

Sometimes, parents may notice a flattened, diamond=shaped area on top of the baby's head ( Fontanelle ) which may appear to pulsate. This is just a normal gap between the scalp bones, and gradually closes as the bones grow and fuse together.

Over the face, one can often see pink birth marks ( Capillary Hemangiomas ) especially over the eyelids and sometimes at the back of the neck too. Folklore has attributed these marks to stork bites as they carry the baby down from heaven ( Which, incidentally, is where they all come from ! )

These are transient, as are fine yellowish dots on the nose ( Milia ) which represent collections of oily substances, and are, by the way, not to be squeezed please !

Up to 70% of normal babies will develop rashes over the body ( Erythema Toxicum ) which are harmless, and typically bother the parents more than it does the baby.

Interestingly, as a consequence of a sudden drop in the mother's female hormones after birth, a few babies experience secretion of breast milk 9 Galactorrhoea ) and even vaginal bleeding ( Withdraw Bleed ) during the first week. Parents should recognize these possibilities and not be unduly alarmed as the effect of these hormones will pass.

The baby's tummy normally appears prominent, especially after a feed. Unfortunately, this leads many caregivers to label the baby's tummy as being " distended with wind ". A general rule of thumb is : if the bay is happy and contented, the tummy is usually not the problem.

Many parents also worry if the cord clamp will hurt their baby - it does not. Within a week after birth, the cord actually dries up into a dark stump, which drops off by itself.

Behaviour / Activity
Babies need sleep to grow well, and in fact, they can sleep as much as 18 hours in a day, waking up only for absolutely pressing reasons like feeding.

When awake and quite, babies love to study their environment, However, their eyes can focus well only up to 20cm. So it is a good idea to get up close and personal to them, because their idea of a good time is to see faces, especially their parents' faces cooing and taking to them.

Babies do not possess the faculty of adult speech ( and they are still unable to help themselves to the fridge ! ), therefore, nature has provided them with the superb set of lungs, Some adults get extremely agitated or depressed when a baby cries. Yet crying does not mean that the parents are not doing a good job, It is the only way for the baby to let you know that they need something. Address those needs, and the crying will stop.

This begs the question : How does one read the language of a baby's cry ?

Most commonly, babies cry because it is feeding time and they are hungry. Or they have soiled themselves with urine, vomitus or stools. Or they are in some kind of discomfort from heat, rashes, insect bites, or colic. Or perhaps they just want a little cuddle ( remember, you can never cuddle little babies too much ! ) For a start, now parents should look for the above possibilities like a checklist, As you spend more time with the baby, you will become better and better at differentiating the cries.

Newborn babies are also preprogrammed with automatic movements ( Reflexes ) that help them protect themselves and cling on to their mothers. Stroke their palms and they will hold on to your finger tightly ( Grasp Reflex ); A sudden noise like a slamming door will cause them to startle, throw their arms up and possibly cry ( Moro's Reflex ).

Feeding
A common question that vexes parents : How much to feed my Baby ?

One good idea is to follow the baby's cues and feed on demand, The amount needed varies because every baby's stomach capacity, metabolic rate and genetic programme is different. And they grow at a terrific rate too !

Breastfed babies may want to feed every 1-2 hours especially in the beginning. Formula-fed babies seem able to last 3-4 hours between feeds, but only because formula is harder to digest !

Whatever it is, remember to give feeding regularly, and also through the night during the first month. ( If baby seems to have slept pass his feeding time, you must gently remind him or her ).

The baby is getting enough if he passes on adequate amount of urine and stools ( see next section ) and putting on weight at a good rate.

Water is not necessary because milk itself contain abundant water. Moreover, water by itself fills up the stomach but provide zero calories and nutrients. After feeding, many babies hiccup due to distention of the stomach and whether or not you do anything about it, hiccups will disappear once the feed is digested.

On the same note, spiting out feeds ( Regurgitation ) is extremely common due to laxity of the newborn's swallowing tube ( Oesophagus ).

While the problem usually resolves within next few months, we can help by periodically burping the baby during and after feeds, not moving him too suddenly , and putting him down on an incline where the head is always slightly higher than the stomach.

Elimination
In the good old days, a sure sign of a new baby was the sight of rows of rows of clothes diapers hanging out to dry. Tobe sure, babies can generate prodigious amounts of waste !

Most babies pass thick, sticky greenish stools ( Meconium ) within 24 hours. With increasing feeds, the stools change colour to yellow within a few days. Because breast milk is easy to digest, bowel movements are usually regular, sometimes occurring with every feed and the stools may appear slightly watery and golden. On the other hand, formula-fed babies may have firmer, bulkier stools, which is passed less often.

Newborn should also pass urine within first 24 hours, and thence, may wet their diapers about 6 to 12 times in a day.

Summary
Knowing what is normal can put a lot of parental anxieties to rest. The best way is to read about babies before delivery, consult family members and friends who have taken care of babies themselves, asking your paediatrician, and most importantly, to learn from observing and being with your baby a lot.

And in between the hard work and interrupted sleep, remember to enjoy and love the little miracle you hold in your arms !

2nd Session : Is My Baby OKAY ?

第一堂课的 1st Session 讲的是以下的资料;

When to see the doctor ?
* Missed period
* Once pregnancy is confirmed by UPT
* Before pregnant if associated with medical disorder

What happens to my body ?
When you are pregnant, many things change in your body to adapt to your baby growing inside you.
These changes often cause some problems for the mother.
However they often do not harm the mother or the baby.
* Morning Sickness
* Constipation
* Heartburn
* Piles
* Cramps
* Vaginal Thrush
* Varicose Veins
* Incontinence

What does the doctor check for when i come to the clinics ?
When the doctor sees you for the first time in the clinic, he often does not know anything about you. He / She needs to talk to you and examine you before the understands you and your pregnancy.
* Weight
* Blood Pressure
* Urine
* Examination
* Ultrasound Scan

Blood Pressure in pregnancy ?
* The normal blood pressure is less than 140/90
* If the blood pressure is raised, usually after 20 weeks, it may mean you have
* pre-eclampsia.
* It is a serious condition which can lead to problems with both the mother
* and baby.

Diabetes in pregnancy ?
* More common in those with family history of diabetes or similar history in
* previous pregnancies.
* May result in preterm deliveries, big baby, too much water around the baby,
* etc.
* Usually blood sugar returns to normal after delivery.

What is the doctor feeling for when he / she examines me ?
* To make sure the baby is growing healthily
* What is the position of the baby ?
* How many babies are there ?
* Has the baby's head got engaged by 36 weeks ?
* Is a vaginal delivery possible ?

How can I prepare myself for labour ?
* Go to Antenatal Class
* Get familiarized with the people and the surroundings
* Get your partner's support
* Exercise regularly
* Take a good diet. Take supplements if necessary

The dos and dos'ts in pregnancy
* Take folic acid during the first trimester. It is even better if you can take it
* before you are pregnant.
* Avoid taking medicine unless it is safe for the baby.
* Exercise
* Wear loose, comfortable clothing, good supportive brassieres and avoid
* high heels.
* Good time to travel will be during the second trimester. Different airlines
* have different rules about how late you can travel by air.
* Try to avoid long distance travelling during the last trimester.
* Sexual intimacy is allowed through the pregnancy unless advised not to by
* the doctor.

Break ~

Friday Night
